Fall protection tops list of OSHA’s most-cited violations for 2025

Recent preliminary numbers from the Occupational Safety and Health Administration show dangers from falls at construction sites remain the most frequently cited hazard on OSHA’s list of most-cited violations for fiscal year 2025, according to Safety+Health magazine.

Other rules related to construction falls are among the top 10 violations, including ladders and scaffolds.

Following are the top 10 most-cited violations for fiscal year 2025:

  1. Fall protection—general requirements at 5,914 violations
  2. Hazard communication at 2,546 violations
  3. Ladders at 2,405 violations
  4. Lockout/tagout at 2,177 violations
  5. Respiratory protection at 1,953 violations
  6. Fall protection—training requirements at 1,907 violations
  7. Scaffolding at 1,905 violations
  8. Powered industrial trucks at 1,826 violations
  9. Personal protective and lifesaving equipment—eye and face protection at 1,665 violations
  10. Machine guarding at 1,239 violations

“While progress has been made, the consistency in citation rankings year after year signals that yesterday’s hazards are still today’s vulnerabilities,” said National Safety Council CEO Lorraine M. Martin. “Employers, safety professionals and communities must intensify efforts through robust training, regular hazard assessments and leadership accountability to protect workers and save lives.”
